Care Services
Knowing what is actually needed
An honest picture of what someone needs now, and what they can keep doing for themselves. Care should be sized to the person, not to the worry.
What an assessment is for
Before anyone moves in, we sit down and build a picture of how they actually live. Not a diagnosis: a portrait. What they can do, what has become difficult, and what they would like to keep doing.
That picture becomes the care plan, and it is revisited as needs evolve. It is also the fastest way to find out that someone needs far less help than their family feared.
